Level of Care Need Determination

Rose Anne Dioguardi has truly been a life saver for both myself and my sister. Living over 3 hours away, I cannot always be on top of issues affecting my sister's life, but Rose Anne is not only on top of issues, she anticipates and avoids most issues. Be it the quality of the caregivers, taking my sister to a doctor's appointment and following up on things, or simply improving the personal surroundings of my sister. Rose Anne can solve the problem more quickly than I ever could. Her sensible, no nonsense, but caring attitude have gained her the trust of my sister, and allowed me to feel assured that I can leave my sister in her home and allow her to age peacefully without being institutionalized. I cannot recommend anyone more highly.

I met Rosanne in 2019 when she was serving as the geriatric care manager for a husband and wife who were at a very difficult stage of their lives and who had no local family to help with the daily medical needs they were requiring. Roseanne reached out to me, as she saw the need for a primary care physician who could provide home visits to this aging couple. Without the extensive services that Roseanne put in place, this couple would have surely been separated, one in a memory care facility and the other likely in a nursing home. They continued to live at home together with the expert help provided by Roseanne. I continue to work with Roseanne and trust that she will provide the most compassionate care and recommendations to my elderly patients.
